I filled out the imformation , name Richard Nemetz, adress; N7778 Hwy 180 Porterfield, Wi. 54159 telephone; 715 732 6381, fax 715 732 6381, email; richardnemetz@yahoo.com, passport don't have one, who issued; never had one issued, date of issue; no date to have. The imformation came back as not filled completely. What else can I do?
